During the climax, John Hurt's character sets Joey up to be garroted with piano wire. When it doesn't work, he looks annoyed and very slowly looks for his gun.

Dude, you KNOW what Joey is capable of. If he gets away you are nearly certain to be killed. So why no panic or immediacy when Joey fights back and starts winning? He's going to kill you! And he does.
