What actors fucked up once and NEVER recovered from it?

And Im not talking about someone like Mel Gibson who screwed up with his tirade but eventually made his way back into good graces. Im talking about actors (usually young and upcoming) who did something that forever ruined their chance to be perceived positively by the public. Bronson Pelletier is one that comes to mind. A side character in the Twilight films he had a promising start but after drunkenly pissing in a busy airport and having the clip played all over tmz and other places I havent heard of him since that.
