I've probably seen Inception 30 times, and just noticed a minor detail that might be a major clue. [spoilers]
In the first sequence (when Cobb is in Saito's dream), he sees Mal and they wind up in one of Saito's rooms. He asks her to have a seat, ties a rope to the leg of the chair, and feels her leg. Cobb then rappels out the window to continue with the extraction. After getting halfway down the wall, we see the chair is now empty, and Cobb falls down the wall because the chair can't support him.
I've always assumed that Mal simply disappeared because she's truly not there and only a vision in Cobb's mind. It just occurred to me that she probably got up out of the chair because she wanted him to fall, because Cobb is truly trapped in a dream, and the fall might be the "kick" he needs to come out of it. If so, this would support the theory that Cobb is truly the one being incepted (i.e.- the entire movie is a dream). I'm sure many others have already spotted this "clue", so sorry if this is old news (I'm a little slow sometimes).
Damn I love this movie...every time I watch it I see something new. But there's no one right answer, almost any theory you throw at it works. It's really the gift that keeps on giving, and IMO it's Nolan's best work.
