What are the most spot-on and casting choices ever? Conversely, what are the biggest casting blunders ever?

Off the top of my head, I would say that picking prime Brad Pitt for the role of Tyler Durden in the adaptation of Fight Club was a perfect choice. On the other hand, regarding not-so-appropriate casting choices, picking Ralph Fiennes for the role of the Tooth Fairy in Red Dragon was just bad. Fiennes is a fine actor, but the character is a bulked-up bodybuilder in the book.
